*The insurance benefits under the Group Accidental Death & Dismemberment and Medical Care Insurance Policy (Form No. G-19000) issued to CDAoA are underwritten by The United States Life Insurance Company in the City of New York , a member company of American International Group, Inc., an A.M. Best rated insurance company, and subject to the exclusions, limitations, terms and conditions of coverage as set forth in the insurance certificate provided in your membership materials. These benefits are provided at CDAoA’s expense to all active members who are enrolled for 30 or more consecutive days as added separate insurance benefits at no additional cost to members.

Coverage is not provided for loss due to a pre-existing condition for 12 months from the Covered Person’s effective membership date.  Coverage is not provided for members over the age of 64.
